In France, companies are required to ensure the good mental and physical health of the people they employ. To this end, the healthcare system provides for regular visits by an occupational physician, a professional specialized in the diagnosis and treatment of work-related pathologies (the name was a clue). His role, which has evolved considerably over the last few decades, is complex and essential for employees in all sectors. Let's take a look at what the occupational physician's jobentails .
The role and main missions of an occupational physician
Medical monitoring of employees
This is his or her main and most important responsibility: caring for French workers. Unlike a general practitioner, an occupational physician'smedical follow-up of employees is regulated by law. This includes, in particular, pre-recruitment check-ups, periodic check-ups, follow-up check-ups and check-ups on request. In all cases, the occupational physician is tasked with assessing employees' suitability for their posts, detecting any work-related health problems at an early stage and, if necessary, referring them to specialized professionals. He can also suggest workstation adjustments or reclassification.
The scope of his work is wide-ranging, as the vast majority of people in the workforce need to see him, at least at the time of hiring. These include employees on :
- CDI
- Fixed-term contracts (e.g. professional training contracts, subsidized contracts),
- temporary employment contracts,
- apprenticeship contracts,
- cesu contracts with private employers
Professional risk prevention
More than any other health professional, the occupational physician plays a key role in preventing occupational illness. To this end, he or she assesses the risks incurred by employees in the course of their work. He is also involved in drawing up and implementing the DUERP (single risk assessment document). He draws on his in-depth knowledge of work environments to identify risk situations and propose corrective action.
In this context, he intervenes above all to counter long-term risks. Careers last a long time, and the repetitive nature of work often leads to physical complications known as musculoskeletal disorders, which can be avoided by implementing simple ergonomic measures. The consequences of neglecting these risks can be serious, ranging from simple work stoppage to permanent physical disability. It is therefore in the occupational physician's best interest to ensure that employers understand what is at stake in the DUERP, and that employees adopt good practices and better postures. This is particularly true in difficult or sedentary jobs.
What's more, recent social awareness has led to the development of a largely underestimated aspect of workers' health: their mental state. Occupational physicians must now work to maintain a healthy work environment and a measured pace, in order to prevent psychological fatigue and the risk of burn-out. Helping companies improve working conditions
Just as the occupational physician supports and protects employees in the context of prevention, he or she can also take concrete action in this area, helping to improve working conditions, whether through the design of workstations or general organization. They provide advice and recommendations, and draw up action plans in consultation with management and employee representatives. This is a very important part of his work, since it enables him to prevent risks and limit the negative consequences for workers.

Education and training to become an occupational physician
Before applying for a job as an occupational physician, you need to complete a long and demanding course of study. It is this rigor alone that produces the best possible doctors. As in most medical specialties, errors must be kept to a minimum, and the occupational physician must be able to adapt to any situation presented to him or her. What's more, the complexity of his work requires him to take an interest in different professional sectors, in order to master their characteristics and subtleties. His or her role therefore goes beyond the medical sphere, and requires solid skills that must be acquired through study. Let's take a look at how to become an occupational physician.
In high school, anyone wishing to become an occupational physician must first obtain a general scientific baccalaureate. While other school diplomas can lead to general medical studies, the scientific stream provides the required foundations. Once this first stage has been completed, the student can move on to the Parcours Accès Santé Spécifique (PASS) or the Licence avec Accès Santé (L.AS). One year later, and subject to success, they can enter the two years of general medical studies common to all specialties. At the end of the course, they will sit the final externship exams (EDN and ECOS), the results of which will give them the opportunity to choose their specialty (in order of priority according to ranking). Once they have embarked on their specialization in occupational medicine, they will have a further 3 years of study under the internship system, enabling them to tackle all aspects of this profession in detail and apply them within French healthcare structures.

The skills and qualities needed to practise
To succeed in this challenging and highly technical career, occupational health physicians need to possess a number of specific qualities, such as
- Medical knowledge : comprehensive medical training, backed up by specialization in occupational medicine, is essential. This includes skills in ergonomics, toxicology, epidemiology, occupational psychology and labor law. Occupational physicians must also be proficient in pathologies related to work environments.
- Ability to analyze work environments: the occupational physician must be able to observe, diagnose and interpret the risks associated with workstations. This involves understanding the physical, organizational and psychosocial constraints specific to each sector of activity.
- Relational and pedagogical skills: active listening, diplomacy, benevolence and the ability to dialogue with different stakeholders (employees, employers, staff representatives) are fundamental. The occupational health physician must also be able to convey prevention messages clearly, appropriately and sometimes persuasively.
- Sense of ethics and confidentiality: respect for medical secrecy and neutrality in exchanges are crucial to establishing a relationship of trust with employees and guaranteeing an independent stance vis-à-vis the employer.
- Ability to work as part of a multi-disciplinary team: occupational health physicians collaborate with occupational health nurses, ergonomists, psychologists, social workers and other physicians. As such, they must be able to coordinate actions, share relevant information and integrate the points of view of all concerned.
- Ability to keep abreast of scientific and regulatory developments: occupational hazards, legal standards and prevention tools are constantly evolving. Occupational physicians must keep abreast of scientific, technical and legal developments in the sector.
- Organizational skills and autonomy: managing visit schedules, prioritizing preventive actions, responding to health or organizational emergencies... the job requires great rigor and the ability to take initiative.

Career prospects for an occupational health physician
Possibility of becoming a coordinating doctor
With several years' experience, an occupational health physician can progress to the position of coordinating physician, or even head physician in larger structures. This role involves managing multidisciplinary teams (doctors, occupational health nurses, ergonomists, psychologists, toxicologists), implementing occupational health policy and guaranteeing the quality of occupational risk prevention actions . It is also a key role in dialogue with senior management and social partners, requiring skills in management, communication and strategy.
Specialization in specific sectors
Occupational medicine offers many opportunities for specialization. Some doctors choose to focus their practice on specific sectors where health issues are of the utmost importance:
- Construction: exposure to noise, falls, musculoskeletal disorders, etc.
- Chemical industry: toxicology, chronic chemical risks.
- Nuclear: radiation protection, rigorous exposure monitoring.
- Transport, logistics, health: working shifts, mental workload, acute stress
These specializations provide in-depth expertise in occupational hazards, often sought after by major industrial groups, government departments, health safety agencies or scientific committees.
Finally, occupational health physicians can opt for an academic career or take part in research projects in collaboration with organizations or universities. Such work may focus on the epidemiology of occupational diseases, the assessment of emerging risks or the development of innovative preventive measures. Some become temporary or permanent lecturers, involved in the training of occupational physicians or in specialized occupational health masters programs.
Participation in occupational health research projects
Occupational physicians can become involved in research, working with specialized institutes or university laboratories. They can contribute to the development of knowledge in occupational epidemiology, ergonomics or preventive innovation. Finally, some practitioners choose to join or set up an independent occupational health department (particularly in large companies), where they can organize tailor-made preventive actions and innovate in terms of quality of working life, social dialogue and individual follow-up.
