Responsibilities
- Conduct psychiatric, psychological, and social assessments at home or in the client’s living environment
- Provide care and clinical follow-up for individuals referred by the EMPP and EPIC
- Intervene early and during acute phases to prevent and defuse crises
- Propose and coordinate alternatives to hospitalization in collaboration with family members and partners
- Serve as a liaison between health and social services and develop collaborative networks
- Participate in the organization and coordination with a CMP or inpatient unit, depending on the candidate’s preferences
