DNA of the structure You will be joining a healthcare cooperation group dedicated to home hospitalization and coordination support, with a family organization and a strong growth dynamic. Our activities mainly cover the Tarbes-Lourdes-Pau-Lannemezan area, and we are developing a territorial mobility pole for palliative care. The company currently employs almost 90 professionals, and is planning to move to larger premises near Ibos in the near future. Digital and traceability tools are made available to secure practices. Description and missions The Advanced Practice Nurse (APN) in HAH provides advanced clinical monitoring, treatment assessment, patient care coordination and complication prevention. Your main tasks will be to - Carry out initial assessment and follow-up consultations at home or via teleconsultation. - Conduct interviews, history-taking and clinical examinations to identify vulnerabilities. - Collect and analyze complex clinical data related to your specialty. - Monitor compliance and side effects of cancer treatments and medical devices. - Adapt and adjust treatments, prescribe and evaluate paraclinical assessments in compliance with regulations. - Implement therapeutic education and intervene in the prevention and management of complications in the home. - Coordinate care paths between hospital, town, patient and family, and take part in communications, staff meetings and RCP. - Participate in professional training, quality audits, research and protocol updates. Assignments may evolve according to the needs of the department. Compensation For this position, you will be remunerated in accordance with agreement 51, executive status + recognition of specialization and expertise (according to coefficient). 208 days worked, 10 days of RTT and an annual bonus equivalent to 5% of annual salary.
