We are recruiting a Médecin MPR M/F to join a homecare facility located in Élancourt as part of a CDIat full time.
Description
As a rehabilitation physician, your main tasks will be to:
- Ensure the implementation of the therapeutic projectof the patientin collaboration with the rehabilitation teamand the Care Department.
- Promote the coordination of healthcare professionals and provide your medical expertise to all those involved.
- Prescribe as part of the guarantee of continuity of care.
- Make scheduled home visits according to the patient's needs.
- Involve you in the daily activity of the rehabilitation unit, including teamwork, valuing services, training professionals, and improving the quality of practices.
- Participate in the institutional life of the establishment, including bodies, transversal development, training or information missions, and partnerships with prescribers and health networks.

Located in Élancourt, our facility offers a dynamic and innovative working environment, with a multidisciplinary team made up of specialized physicians, physiotherapists, nurses, social assistants, psychologists. Equipment at your disposal includes modern rehabilitation equipment, telemedicine tools for remote coordination, and service vehicles for home travel. In addition, the full hospitalization zone ensures 24/7 continuity of care in the Yvelines department, with the exception of the Mantois sector.
