Structure DNA You will be joining a non-profit, mutualist EHPAD in Nantes, dedicated to supporting the frail elderly. It accommodates 86 residents, and places great emphasis on the quality of care provided and the caring attitude of the team. In addition, it is ideally located in Saint-Nazaire, a modern, pleasant city with all the shops and services needed, offering a practical and accessible living environment for professionals and families. Description and responsibilities You will report to the Director of the EHPAD and work closely with the medical and quality of care departments. Your main tasks will be : Organization and coordination of care - Ensure quality and continuity of care - Coordinate in-house teams and private practitioners - Organize geriatric commissions - Ensure good geriatric practice and the appropriateness of prescriptions Resident admission and follow-up - Assess the suitability of health status and care capacities - Carry out AGGIR and PATHOS assessments - Participate in drawing up and monitoring individualized care plans Institutional dynamics - Participate in ongoing projects - Reorganize meal times - Developing prevention skills and identifying frailties - Implementation of EGS Strengthening the role of families - Contribute to team cohesion and improved working conditions - Participate in strategic reflection on the development of the establishment Compensation For this position, you will benefit from a remuneration of between €70,000 and €100,000 gross for a full-time equivalent, according to the FEHAP grid.
