The Facility
This is a medical care facility located in Soisy-sous-Montmorency, serving fifty-five residents, including forty-nine residential beds and six day-care spots. The residents have intellectual disabilities but no severe medical conditions, and the facility has two specialized units: one for the elderly and the other for neurodevelopmental disorders and autism spectrum disorders. The position, which focuses on liaison and coordination, is part of a stable and supportive team comprising nurses, nursing assistants, medical-psychological counselors, a psychomotor therapist, a physical therapist, a psychologist, a physician, and the head of nursing services. Furthermore, the workload is manageable, with an organizational structure that allows for a ratio of approximately one nurse to fifteen patients. Located in the Val-d’Oise region at the foot of the Montmorency Forest, the town offers a green, residential setting just a few minutes from Paris and well-served by public transportation.
Compensation
For this position, your compensation will be based on the CCN51 pay scale, with seniority credit granted according to applicable conditions.
Responsibilities
- Serve as the liaison between nursing teams, doctors, families, and partners
- Coordinate with the physician to renew prescriptions and follow up on medical appointments
- Serve as a liaison with residents’ families
- Develop and coordinate partnerships with healthcare providers, home health care (HAD), and mobile teams
- Serve as a liaison with care providers
- Ensure the smooth coordination of residents’ care pathways
