DNA of the structure You'll be working in an EHPAD set in 17 hectares of parkland, easily accessible from the A140 Francilienne motorway and by public transport from Chelles. What's more, in 2023, the facility will be relocated to new, eco-labeled premises, designed to promote residents' well-being. All units have en-suite bathrooms, some are interconnected, and 23 rooms are equipped with transfer rails. The building is also adapted for people with reduced mobility, with two elevators and wide circulation areas. The establishment promotes an approach focused on residents' autonomy and quality of life. Description and duties You will be in charge of medical coordination and supervision of the care process within the facility, up to 0.5 FTE. Your main responsibilities will include - Participate in drawing up and monitoring individualized support projects for residents. - Ensure coordination and consultation between professionals on the multidisciplinary team. - Collaborate with complementary care services within the establishment, in particular the HAD and a specialized palliative care team. - Contribute to actions aimed at improving the quality of practices and risk prevention. - Participate in exchanges with family and friends, and in internal bodies linked to the establishment's project.
