An EHPAD in Poissy, in the Yvelines, is looking for its new coordinating doctor M/F for a part-time CDI. Terms and conditions - Part-time permanent contract - 4 days a week - 7h00 a day The structure This is a nursing home for dependent elderly people located in Poissy, in the heart of an urban and residential area. The care teams are structured around a prescribing doctor, an IDEC team manager and a nursing team responsible for the daily care of residents. In addition, the site boasts a large number of care assistants, enabling enhanced care coordination and support focused on residents' dignity and well-being. Your salary - 8,000€ gross/month (excluding SEGUR) Missions 1. Ensure good geriatric practice -Coordinate geriatric assessments and formulate diagnostic and therapeutic proposals, both medicinal and non-medicinal. - Promote best practice in drug prescription for the elderly. - Ensure the dissemination and application of geriatric recommendations and protocols. - Contribute to the evaluation of care quality. - Participate in the implementation of training and information initiatives for professionals. - Participate in drawing up, coordinating and monitoring residents' personalized care plans. 2. Guarantee residents' rights, safety and support - Act as the primary contact for residents, their families and, where applicable, their legal representatives. - Guarantee individual freedoms and respect residents' rights. - Develop and reassess measures to restrict freedom when strictly necessary (restraint, protected unit, additional precautions, etc.). - Ensure that measures taken are consistent with the personalized project. - Guarantee the physical integrity, safety and freedom of movement of residents. - Provide optimum support for residents in palliative care. 3. Participate in the development and implementation of the care project - Work with the nursing team to draw up the general care project as part of the facility's overall plan. - Coordinate and evaluate the implementation of the care project. - Analyze admission requests and give a medical opinion on the match between the resident's state of health and the facility's care capacities. - Assess and validate dependency levels and care needs (AGGIR / PATHOS). - Raise teams' awareness of regulatory requirements for keeping resident files. - Draw up and coordinate the annual medical activity report (RAMA), co-signed with the facility's management. 4. Guarantee continuity of care - Streamline the resident's care pathway and monitor chronic pathologies. - Ensure appropriate care in emergency situations and prescribe the necessary therapies. - Provide care when no doctor is available for consultation. - Ensure the quality and completeness of medical records. - Organize care in the event of exceptional risks. - Develop and promote telemedicine. 5. Promote prevention and public health initiatives - Identify public health risks and organize preventive actions within the facility. - Participate in the continuous improvement of care quality and safety. - Control infectious risks and ensure the safety of the medication circuit. - Participate in and lead the RETEX process relating to serious adverse events. 6. Contribute to the establishment's reputation - Organize and chair the Geriatric Coordination Commission. - Develop partnerships with local healthcare players. - Participate in the implementation of agreements with healthcare establishments and professionals. - Participate in the supervision of interns and medical students. 7. Participate in national missions - Occasionally contribute to national work, bodies or missions under the aegis of the Direction Médicale France.
