In this age of sedentary lifestyles, our bodies are suffering. After a certain age, it's not uncommon to need medical intervention for the musculoskeletal system - the set of organs that enable us to move around. This is when the role of the orthopedist comes into its own. This specialist in bone and joint pathologies treats patients suffering from deformities, so that they can regain their strength and autonomy. It's a difficult and highly technical profession, which we're going to try to decipher in this new job description.

The missions of an orthopedist

As explained in the introduction, an orthopedist (also known as an orthopedic surgeon) is a doctor specialized in the diagnosis, treatment and prevention of disorders of the musculoskeletal system. His field of action encompasses bones, joints, muscles, tendons, ligaments and the peripheral nervous system. In view of France's ageing population and the increase in sporting activities, this profession, which is experiencing a growing demand for orthopaedic surgeons, requires the specialist to fulfil a wide range of important missions, which we propose to detail below.

Diagnosis of musculoskeletal disorders

This is the preliminary stage in any specialist's work, and therefore in that of the orthopaedist. This analysis phase requires advanced skills in anatomy, as well as sound clinical expertise. The orthopedist performs an examination to identify the patient's symptoms. They also test mobility and muscle strength in the light of the patient's medical history. To take things a step further, he or she may also carry out a full assessment of locomotor functions, range of motion, joint stability and neurological reflexes.

Following this collection of data and information, the orthopedist analyzes X-rays, scans, MRIs and arthroscopies to identify possible lesions. Thanks to his mastery of the reading of different medical imaging media, he can draw up a therapeutic plan adapted to each pathology.

Medical and surgical management of patients

Once the pathology has been identified, the orthopedic surgeon prescribes treatment. Whenever possible, non-invasive treatments are preferred: anti-inflammatories, infiltrations, physiotherapy or orthotics. If not, he prescribes surgery. A whole process then ensues, in which the orthopedist must explain to the patient the expected benefits, potential risks and therapeutic alternatives. Pre-operative preparation includes assessing the patient's general condition, scheduling anesthesia and organizing the logistics of hospitalization. This preparatory phase largely determines the success of the operation.

Then there's the operation itself, the heart of the orthopedist's profession. To ensure that the operation runs smoothly, the orthopedist must master all orthopedic surgical techniques. In a constantly evolving medical world, this is no mean feat! The medical world is constantly evolving, and new technologies are regularly introduced. The orthopaedic surgeon must therefore keep abreast of the latest innovations and master modern techniques such as arthroscopy, computer-assisted surgery, surgical robotics and complex reconstruction techniques.

Post-operative follow-up and rehabilitation

Once the operation has been completed, the orthopaedist's work is not finished. To ensure that the patient recovers, post-operative follow-up is almost as important as the previous stages. During this phase, the specialist monitors the healing process, prevents infectious complications and adjusts the rehabilitation protocol according to any changes observed. This regular monitoring enables any complications to be detected and treatment adapted accordingly.

Finally, coordination with physiotherapy teams is a fundamental aspect of orthopedic follow-up. The orthopedist prescribes personalized rehabilitation protocols, defines the functional objectives to be achieved and adjusts progress according to the patient's abilities. This multidisciplinary collaboration optimizes motor recovery and prevents long-term sequelae.

Areas of intervention in orthopedics

Pediatric orthopedics

This orthopedicspecialty deals with disorders of the musculoskeletal system in children and adolescents, taking into account growth and motor development. It treats congenital pathologies (hip dislocation, vertebral malformations), growth disorders (scoliosis, length inequalities) and traumas specific to this population category. He also adapts his techniques to preserve growth cartilage and limit after-effects.

Sports traumatology

With the boom in physical activity, sports traumatology is a rapidly expanding field. Orthopaedic surgeons treat injuries specific to each discipline (ligament ruptures, tendinopathies, fatigue fractures). They work with sports professionals to ensure a rapid and safe return to competition.

Degenerative pathologies

Age-related conditions such as osteoarthritis and spinal disorders account for a significant proportion of orthopaedic activity. Treatment begins with conservative solutions (infiltrations, re-education), but can progress to prostheses (hip, knee, shoulder) in the case of advanced damage. Spinal pathologies are treated in collaboration with other specialists, and require advanced technical expertise.

Education and training to become an orthopaedic surgeon

To become an orthopaedic surgeon and apply for a job as an orthopaedic surgeon, you need to follow one of the most demanding academic paths in the French medical system. This rigor is essential: orthopedists work directly on the human skeleton, sometimes operating in emergency situations where the slightest technical error can have lasting functional consequences (i.e. permanent disability). Surgical gestures must therefore be absolutely precise, implants perfectly positioned, and each operation anticipated through rigorous planning. To achieve this level of expertise, orthopaedic surgeons must undergo a demanding training program that enables them to adapt to a multitude of anatomical, traumatic and degenerative situations.

It all starts with a general baccalaureate with a focus on science. Although other routes are theoretically possible, the scientific stream is the best preparation for medical studies. Once you've passed your baccalauréat, you'll need to pass the Parcours Accès Santé Spécifique (PASS) or a Licence avec Accès Santé (L.AS), which will enable you to enter medical school. The first cycle (3 years, including PASS) gives access to the Diplôme de Formation Générale en Sciences Médicales (DFGSM) and introduces the fundamentals of medicine.

Students then enter the externat, a 3-year second cycle divided between theoretical courses and hospital internships. Finally, the young graduate must sit the final externat exams (EDN and ECNOS), the results of which will enable him or her to choose a specialty.

Specialization in orthopaedic surgery then lasts 5 years, in the form of an internship in various surgical departments. Here, students learn different surgical techniques, management of musculoskeletal pathologies, and diagnostic and post-operative approaches. At the end of this course, they are awarded the Diplôme d'Études Spécialisées (DES) in Orthopaedic and Traumatological Surgery.

Working conditions and working environment

Possible working environments

Throughout their career as orthopaedic surgeons, these doctors may work in a variety of environments (and may, to be helped, go through a medical recruiter). The tasks they perform vary slightly from one structure to another:

Public and private hospitals

Hospitals, whether public or private, are the main places of practice for orthopedists. In this context, they:

➔ operate in equipped blocks and work with multidisciplinary teams
➔ participate in training, research or regional referral programs

Specialized surgical clinics

Private medical practices

Some orthopedists choose to practice in private practice, either alone or in a group. In this case, they:

➔ manage the administrative, logistical and financial aspects of the practice themselves

Mixed practice

Many orthopedists combine several practice environments. They can:
