The Facility
You will join a healthcare facility with multiple locations throughout the department. The work primarily involves follow-up care and rehabilitation with a focus on neurology, geriatrics, and musculoskeletal conditions. Additionally, the organization encourages a diverse range of activities and multidisciplinary coordination.
Compensation
- Compensation based on the CCN 51 pay scale

Responsibilities
- Conducting speech-language pathology evaluations
- Developing and implementing rehabilitation programs tailored to SMR patients
- Supporting patients in neurology, geriatrics, and musculoskeletal disorders
- Working as part of a multidisciplinary team and participating in review meetings
- Maintaining patient records and sharing information with the care pathway
