Job description and duties
The aim of the position is to provide social support for patients during their hospital stay, and to assist them with administrative and discharge procedures. Your main tasks will be to
- Inform, evaluate and support the implementation of an individualized assistance plan
- Coordinate the players involved in supporting the patient
- Develop collective information and prevention initiatives for patients and professionals
- Monitor local needs and resources
Your day-to-day activities will include
- Carrying out social interviews following admission assessment or social pre-diagnosis
- Analyze needs and propose appropriate discharge solutions (return home, transfer to EHPAD, transfer to other services or structures, request for legal protection)
- Compile files relating to legal protection measures
- Present and promote the group's services to patients and their families
- Participate in transmission meetings and ensure traceability of social actions in the patient file
- Contribute to quality initiatives, audits and working groups aimed at improving the organization of discharges
