The Facility
This home-based rehabilitation facility is located in Amilly, in the Loiret department, but also serves part of the surrounding area. Its team is collaborative and caring, consisting of at least two rehabilitation specialists (physical therapists, occupational therapists, psychomotor therapists, speech-language pathologists, and dietitians) as well as nursing and nursing assistant staff for standard home hospitalization care. In addition, the service has a shared office for the team, small rehabilitation equipment, and organizes regular team meetings and multidisciplinary staff meetings.

Responsibilities
- Conduct psychomotor assessments and develop individualized treatment plans
- Provide psychomotor therapy sessions at patients’ homes as part of a multidisciplinary rehabilitation program
- Participate in staff and team meetings to coordinate care plans
- Use and maintain the available rehabilitation equipment
- Write reports and handoff notes to ensure continuity of care
