You will work within a seamless organizational structure, in close collaboration with general practitioners and specialists to coordinate the patient care pathway. Compensation
For this position, your compensation will be determined based on your profile and experience.
Responsibilities
- Follow-up consultations for patients with stable chronic conditions
- Physical examinations and assessment of patients’ health status
- Monitoring patients with multiple conditions and helping to coordinate their care
- Collaborating with physicians and referring patients back to their doctor when necessary
