We are recruiting a General Practitioner M/F to join a health establishment located in Siouville-Hague (Manche) as part of a CDIà full time.

Description
As a general practitioner, you will provide careto patients while guaranteeing their continuity. You will actively participate in the development and implementation of the establishment's projects and the medical project. Your missions will also include the continuous improvement of quality and risk management, as well as the integration of the establishment into the various care channels. You will also be responsible for:
- Leading weekly multidisciplinary staff meetings in line with IPAQSS
requirements. - Drawing up the therapeutic project (care project, life project) for patients as part of a multi-professional team, and presenting it to the patientand/or trusted person
. - Organize the discharge of patients, ensuring the relevance of the referral and the support system in place
. - Write the liaison letter, give it to the patient on the day of discharge and send it to the physicians taking care of the patient
. - Carry out medication reconciliation for patients prioritized by the facility
. - Engage in the proper maintenance of patient records on Hospital Manager (HM) to ensure optimal traceability reflecting the care management of the patient and meet regulatory requirements through the monitoring of indicators
(IPAQSS, KPI)
- Perform weekly PMSI coding of patients and train in coding where necessary

Profiles sought: General practitioner M/F holding a doctorate in general medicine in France, registered or registrable with the Conseil national de l'ordre des médecins in France, holding the DES, DU or capacité that corresponds to the specialty of the structure, with prior experience in town medicine or hospital structure.
